ABSTRACT

OBJECTIVE To analyze the patterns and characteristics of drug-related administrative penalty cases with medical institutions as parties from 2020 to 2022 in order to further improve drug management in medical institutions. METHODS A retrospective statistical analysis was used to summarize the drug-related administrative penalty decisions with medical institutions as parties, and to match them with the provisions of the Drug Administration Law (2019 version) for statistical analysis. RESULTS There were 144 complete administrative penalty decisions with medical institutions as parties. Analyzed by cause, 126 cases of administrative punishment for inferior drugs accounted for 87.50%, of which expired drugs accounted for more than 50.00% of the inferior drug cases; 15 cases (10.42%) were for purchasing drugs from enterprises or individuals not qualified to operate drugs. Analyzed by the range of punishment amount of the cases, 34 cases (23.61%) resulted in lighter penalties, while 81 cases (56.25%) resulted in reduced penalties. CONCLUSIONS There are extremely few medical institutions that have received administrative penalties for drug management violations. Medical institutions should strengthen the awareness of law-abiding, and know the red line of drug management and the illegal behavior that is easy to occur, so as to better strengthen drug quality management.

ABSTRACT

Objective:To explore the clinical efficacy of lenalidomide combined with second-line immunochemotherapy as a salvage regimen in the treatment of relapsed/refractory diffuse large B-cell lymphoma (DLBCL).Methods:The clinical data of 37 relapsed/refractory DLBCL patients receiving lenalidomide combined with second-line immunochemotherapy as a salvage regimen who had recurrence after autologous hematopoietic stem cell transplantation or who were not eligible for transplantation or had no intention to transplant between January 2016 and December 2020 in the First Affiliated Hospital of University of Science and Technology of China were retrospectively analyzed. Among 37 patients, 6 cases with primary central nervous system (CNS) lymphoma and 3 cases with secondary CNS lymphoma. The short-term efficacy after treatment was evaluated. Kaplan-Meier method was used to analyze the overall survival (OS) and progression-free survival (PFS), and log-rank test was used for subgroup comparison. Results:The median follow-up time of 37 patients was 20.4 months (2.7-37.0 months). At the end of treatment, the overall response rate (ORR) of all patients was 64.9% (24/37), the complete response (CR) rate was 45.9% (17/37), and the median duration of response (DOR) of 24 patients who responded to treatment was 17.7 months (3.6-33.6 months). The median PFS time of all patients was 11.2 months, and the 1-year PFS rate was 48.6% (95% CI 32.5%-64.7%). The median OS time of all patients was not reached, and the 1-year OS was 67.6% (95% CI 52.5%-82.7%). Among 24 responding patients, 17 cases who received lenalidomide maintenance therapy after remission tended to have a better response compared with 7 cases who did not receive lenalidomide maintenance therapy after remission, although there was no significant difference in OS and PFS between both groups (both P > 0.05). Additionally, neutropenia was the most common adverse reaction with an incidence of 81.1% (30/37). Conclusions:Lenalidomide combined with the second-line immunochemotherapy may be an effective salvage therapy for patients with relapsed/refractory DLBCL, especially for patients with CNS involvement. The patients achieving remission after salvage therapy continue to receive lenalidomide maintenance therapy and could have a better prognosis.

ABSTRACT

BACKGROUND: Chemotherapy, local radiotherapy, autologous peripheral blood stem cell transplantation and cellular immunotherapy are the treatment options for lymphoma. High-dose chemotherapy combined with autologous hematopoietic stem cell transplantation can significantly prolong the survival time and improve the prognosis of patients. It is recommended as the first-line treatment for relapsed refractory and (or) highly invasive lymphoma. OBJECTIVE: To explore the influencing factors of high-dose chemotherapy combined with autologous hematopoietic stem cell transplantation in lymphoma. METHODS: The clinical records of 74 lymphoma patients after high-dose chemotherapy combined with autologous hematopoietic stem cell transplantation in Transplantation Ward, Department of Hematology, West District of The First Affiliated Hospital of University of Science and Technology of China from October 2015 to March 2020 were collected and analyzed retrospectively to evaluate efficacy and prognostic factors of autologous hematopoietic stem cell transplantation for lymphoma. RESULTS AND CONCLUSION: (1) The follow-up period was up to May 15, 2020. The median time from diagnosis to transplantation was 8(3-83) months, and the median follow-up time was 17(2-59) months. (2) All patients obtained hematopoietic reconstruction after transplantation. The median time for granulocyte implantation was +10(+8-+17) days, and the median time for platelet implantation was +12(+9-+22) days. (3) There were 60 cases of progression-free survival after transplantation, 13 cases of recurrence, 11 of the relapsed patients died, and 1 died of lung infection 11 months after transplantation. (4) All four patients with progression disease before transplantation died within 7 months after transplantation due to the progression of the primary disease. (5) The 2-year overall survival rate after receiving autologous hematopoietic stem cell transplantation was 78.5%; the 2-year progression-free survival rate was 75.8%. Patients with international prognostic index ≤ 2 points before transplantation and international prognostic index > 2 points had 93.9% and 66.4% overall survival at 2 years after transplantation (P=0.003); progression-free survival rates at 2 years were 85.6% and 65.5% (P=0.017), respectively. (6) The two-year overall survival rates of patients with bone marrow invasion and no bone marrow invasion before transplantation were 55.5% and 91.9% (P=0.001) respectively. The 2-year progression-free survival rates were 53.1% and 88.7% (P < 0.001), respectively. (7) The 2-year overall survival rate of patients in clinical staging (stages I and II) was better than that in clinical staging (stages III and IV) (100% vs. 82.5%, P=0.026). The 2-year progression-free survival rate of first-line consolidation patients was better than that of rescue group (84% vs. 48.9%, P=0.01). There was no statistically significant difference in the effects of patient age and degree of remission before transplantation on progression-free survival and overall survival. (8) Results found that high-dose chemotherapy combined with autologous hematopoietic stem cell transplantation could significantly improve the survival and prognosis of patients with lymphoma, and had high safety. It can be used as a safe and effective treatment for lymphoma. International prognostic index score, the presence or absence of bone marrow invasion, the timing of transplantation, and the stage of primary disease are relative to the prognosis of involved patients.

ABSTRACT

Hypoxia, a salient feature of most solid tumors, confers invasiveness and resistance to the tumor cells. Oxygen-consumption photodynamic therapy (PDT) suffers from the undesirable impediment of local hypoxia in tumors. Moreover, PDT could further worsen hypoxia. Therefore, developing effective strategies for manipulating hypoxia and improving the effectiveness of PDT has been a focus on antitumor treatment. In this review, the mechanism and relationship of tumor hypoxia and PDT are discussed. Moreover, we highlight recent trends in the field of nanomedicines to modulate hypoxia for enhancing PDT, such as oxygen supply systems, down-regulation of oxygen consumption and hypoxia utilization. Finally, the opportunities and challenges are put forward to facilitate the development and clinical transformation of PDT.

ABSTRACT

OBJECTIVES: To investigate the distribution of serum calcium and the relationship between serum calcium and serum metabolic parameters in endometrial carcinoma (EC) patients. METHODS: Retrospective assessment of patients diagnosed with endometrial cancer from Peking University People's Hospital from 2004 to 2009. Clinical characteristics as well as pretreatment serum calcium, albumin, fasting plasma glucose (FPG), serum triglycerides (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL), and total cholesterol (TC) value were extracted from patient records. Serum calcium was corrected for albumin. Unpaired t test and analysis of covariance were used to compare serum calcium among categorical variables. Simple correlation analyses and partial correlation analyses were used to assess the associations between serum calcium and continuous variables. RESULTS: Two-hundred twenty patients were included in this study. After adjusting for confounders, postmenopausal patients had higher total serum calcium (p=0.002) and albumin-corrected serum calcium (p=0.012) than premenopausal patients, endometrioid endometrial carcinoma (EEC) patients had higher total serum calcium than non-endometrioid endometrial carcinoma (NEEC) patients (p=0.037). Significant positive correlations were found between total serum calcium and FPG (p=0.017), TG (p=0.043), HDL (p=0.042), LDL (p < 0.001), and TC (p < 0.001) after adjusting for multiple variables, and the corrected serum calcium showed no significant correlation with metabolic parameters. CONCLUSION: Total serum calcium might be a more sensitive parameter for metabolic syndrome in endometrioid endometrial cancer patients than lipids.
